Add attributes and apply backend result in server monitoring

The code has been updated to include the creation of attributes and a new backend result within the server monitoring. The attributes for "Proxy-Host" are assigned with the server IP, and applied to the backend result. These procedures are implemented for both the current and new backend servers.
